YourCare Health Plan Inc., a not-for-profit health plan, provides managed care insurance to residents eligible for Medicaid, Child Health Plus, and other government sponsored programs in Erie, Chautauqua, Cattaraugus, and Allegany counties. It provides Medicaid Managed Care and Child Health Plus health plans. YourCare Health Plan Inc. was formerly known as Univera Community Health, Inc. As a result of acquisition of Univera Community Health, Inc. by The Monroe Plan For Medical Care, Inc., Univera Community Health, Inc.'s name was changed to YourCare Health Plan Inc. The company was founded in 1995 and is based in Pittsford, New York. YourCare Health Plan Inc. operates as a subsidiary of The Monroe Plan For Medical Care, Inc.

YourCare serves individuals and families who need health care coverage and may not have access to or be readily able to afford employer sponsored or direct pay health insurance.  YourCare has members who are covered by two state government programs which are often referred to as safety net programs for their role in helping vulnerable populations.

Medicaid Managed Care:  Is available to eligible individuals enrolled in Safety Net (formerly Home Relief), Temporary Assistance to Needy Families (TANF, formerly Aid to Dependent Children), and Supplemental Security Income (SSI).

Child Health Plus:  Covers children who are not eligible for Medicaid; for those with family income up to 250% of the federal poverty level, the monthly premium is subsidized.
